At SophAri Care, we are a team of dedicated NDIS providers committed to delivering high-quality, person-centred care. With backgrounds in nursing and disability support, we bring a wealth of experience and compassion to the services we offer. Our team has extensive experience in disability support work, building strong relationships with participants and focusing on their well-being and independence. With a background in registered nursing, we offer expert care, particularly in complex care settings, always prioritising respect and professionalism.

Assistance with Daily Living

Assistance with Daily Living (ADL) is a key support category under the National Disability Insurance Scheme (NDIS), designed to help participants carry out everyday personal tasks and routines they might not be able to do independently due to their disability. Our care support team helps with these and many more:

  • Personal Care: Help with showering, dressing, grooming, and hygiene.
  • Meal Support: Assistance with cooking, eating, and mealtime routines.
  • Household Tasks: Cleaning, laundry, gardening, and home safety.
  • Mobility: Support moving around, transfers, and public transport.
  • Daily Management: Help with finances, appointments, and assistive tech.

Community Access

The NDIS recognises that social and community participation is vital for individuals with disabilities. Engaging in community activities can enhance a sense of belonging, build confidence, and improve mental health. Supports are tailored to help participants connect with others and participate in activities that align with their interests and goals. Some activities include:

Socialisation and Group Activities

NDIS supports social inclusion through group activities, helping participants develop social skills, independence, and overall well-being. These activities are designed to promote social interaction and community engagement in a supportive environment. The NDIS may fund support for attending social groups, personal development courses, outings, and more. Some activities include:

  • Recreational Activities: Group-based arts, music, sports, etc.
  • Educational Programs: Skill-building to support independence and well-being.
  • Outings & Holidays: Support to attend trips, events, or holidays.

Complex Nursing Care

Types of NDIS Nursing and Complex Support services provided by HCA
NDIS Nursing and Complex Support. We can assist in your transition from hospital to home or residential facility, along with your ongoing nursing and complex support needs. Services are provided under the current NDIS price guide and enable you to access multiple support services with SophAri Care includes:

  • Medication Management: Safe support with daily medications.
  • Diabetes Care: Monitoring and assistance with diabetes management.
  • Specialised Care: PEG feeding, continence, ventilator, and bowel care.
  • Assessments: Continence assessments and holistic care planning.
  • In-Home Health Support: Ongoing daily health care in your home.

The NDIS (National Disability Insurance Scheme) is Australia’s funding program that helps eligible individuals under 65 with permanent disability access the supports and services they need to meet their goals.

If you’re not satisfied, you can switch providers at any time. You’re in control—and you can choose a provider that better fits your needs.

Eligibility depends on four things:

  1. Age – You must be under 65 when applying.
  2. Residency – You need to be an Australian citizen, permanent resident, or NZ citizen with a valid visa.
  3. Disability – Your disability must significantly affect your daily life.
  4. Early intervention is also possible if it can greatly improve your long-term independence.

The NDIS pays for supports that are “reasonable and necessary” to help you achieve your personal goals and improve independence, including therapy, community access, daily living support, equipment, and more.

Yes—family or carers can be included as much or as little as you prefer in your planning, meetings, and ongoing support.

  1. Home Modifications: Yes—if they’re reasonable, necessary, and for your primary home (e.g., ramps, bathroom modifications), and other options (like equipment) aren’t suitable.
  2. Transport: Yes—NDIS may fund transport if you’re unable to use public options, or require support to develop transport independence.
